HaysKids School of Transitions Forms
This is a program that we have developed and are using at HaysKids. We adapt it according to a child’s special needs. This appears to be helping our kids developmentally. Our hope is that it will give them tools to overcome the devastating effects of their disability. This program is helping them function at a higher level in our home. It is helping us as parents be calmer and give us hope. As one child said, “Now I know exactly what’s expected and where I stand.” Adapt it, change it to meet your needs.
