		<title>HaysKids School of Transition</title>
		<description>This is a program that we have developed and are using at HaysKids.  It appears to be helping our kids to have developmental growth.  Our hope is that it will give them tools to overcome the devastating effects of their disability.  It is helping them function at a higher level in our home.  It is helping us as parents be calmer and give us hope.  As one child said, “Now I know exactly where I’m at.”  Adapt it, change it to meet your needs.    HaysKids School of Transitions                                              These are forms which we use at HaysKids for the School of Transition.
